What this website
collects, and why
This notice covers ns.mmcwellness.ca. It says what is collected, what it is used for, who else receives it, and how to decline. It is written as a description of what the site does, not as a set of promises.

Who is responsible
MMC Anti-Aging @ North Shore, 101-1133 Lonsdale Ave, North Vancouver, BC V7M 2H4. Part of MMC Wellness Group.
North Shore Plastic Surgery shares this address and is a separate company with its own privacy practices. If you ask us to arrange a surgical consultation we have to pass your details to them; nothing else on this website reaches them.
The patient portal at ns-my.mmcwellness.ca is a different application and is not described here.
If you fill in the enquiry form
- Your name.
- An email address, a phone number, or both. At least one is required, because otherwise there is no way to answer you.
- Whatever you wrote in the message field.
- The page you were on when you opened the form, and the treatment that page was about.
- Your preferred language.
- "How did you hear about us", if you answer it. It is optional and it has a "prefer not to say" option.
- Campaign parameters carried by the link you arrived on (utm_source and similar), if there were any.
What is sent to the clinic is:
If you book online
- Your first and last name.
- An email address and a mobile number.
- The service, the practitioner and the time you chose.
- Anything you typed into the notes field.
The booking form sends:
What is collected whether or not you do anything
- A Google Analytics script loads on every page. It records the address of the page, the page you arrived from, your browser and device type, an approximate location Google derives from your network address, and an identifier stored in your browser so that repeat visits are counted as one person.
- The page addresses on this site name health topics — /en/concerns/always-tired is one of them. The subject you were reading is therefore part of what is sent to Google.
- When you submit a form, a Cloudflare check runs to judge whether the submission is automated. Your network address and a token are sent to Cloudflare for that check.
- The web server keeps an ordinary request log: network address, time, page requested and the browser identification string. Cloudflare, which sits in front of this site, keeps its own equivalent.
This site carries no advertising and no third-party tracker other than the Google Analytics script described above.
What it is used for
- Answering what you asked.
- Holding, confirming and preparing for an appointment.
- Keeping the site working and recognising automated abuse of the forms.
- Seeing which pages are read and which are not.
Who else receives it
- Google LLC — the analytics described above.
- Cloudflare — delivery of this site, and the automated-submission check on the forms.
- Resend — used only when the clinic's records system cannot be reached: a copy of your enquiry is emailed to the clinic through that provider so the enquiry is not lost.
Information handled by these companies may be stored or processed outside Canada. While it is outside Canada it is subject to the laws of the country it is in, and may be reachable by the courts and authorities of that country.
How long it is kept
An enquiry or a booking becomes a record in the clinic's patient management system and is kept under the clinic's records policy and the retention periods British Columbia law sets for records of that kind.
Analytics data is held by Google under the retention period configured on the analytics property. This website keeps no separate copy of it.
Web server and Cloudflare request logs are kept for a short operational period and then discarded.
Declining
- This website does not show a cookie banner, and the analytics script loads on every page.
- It does not act on a Do Not Track or Global Privacy Control signal from your browser.
- Cookies can be blocked or deleted through your browser's settings.
- Google publishes a browser add-on that stops Google Analytics collecting anything, on any site.
- The enquiry form and the booking form are the only places this website asks you for anything. Not filling them in keeps that information off our systems entirely.
Your rights in British Columbia
The Personal Information Protection Act gives you the right to ask what personal information an organisation holds about you, how it has been used, and who it has been disclosed to, and to ask for corrections to it. Send that request by post to the address above, or through the contact form on this site.
The Act also allows a complaint to be made to the Office of the Information and Privacy Commissioner for British Columbia.
